If you're only wearing underwear so that it's not under anything, is it simply called "wear"?

Actually, Superman's "underwear" was really outerwear. Jerry Siegel and Joe Shuster modeled his costume after a circus strongman's. They wore these tight singlets, and would often wear a pair of shorts over top just in case a wardrobe malfunction happened while they were lifting their weights. A contemporary reader would have understood the context and not seen "underwear."
